Question
Posted on: March 6 2013I got following preparation from NKO arts:
Polymyxin B 100,000 E
chloramphenicol 50mg
Propylene glycol 5ml
Water 5ml
Hydrocortisone sodium phosphate 50mg
chloramphenicol 50mg
Propylene glycol 5ml
Water 5ml
Hydrocortisone sodium phosphate 50mg
fac ear drops, sterile prepare
S/3 x 2 dr/day both ears for 7 days < br/> Opm: Hydrocortisone Sodium phosphate no longer exists: possibly replaced by acetate, such as formula mentioned on your website?
question: Sterile preparation? It is for a child with Diabolos: so perforated ear!!
Answer
- as you write: Sterile prepare
- Ask the doctor if hydrocortisone after phosphate may be replaced by prednisolone or dexamethasone after phosphate, both of which are also soluble in water. Hydrocortisone acetate requires you to prepare a sterile suspension
- Propylene glycol is not suitable for the inner ear and should be removed so that I fear that 50 mg of chloramphenicol no longer dissolves unless in the presence of boric acid. But these 2 products are unlikely to be recommended for the inner ear. You could suggest to the Doctor neomycin sulfate to replace chloramphenicol.
I end with a reference to a composition, which is published in the TMF 2 ° part.
R/Polymyxin B sulfate 500,000 IU Dexamethasone sodium phosphate 36mg Neomycin sulfate 250mg Propylene glycol 10g Excipiens AD 50ml
The composition of the excipient is:
Citric acid 1aq 112.5 mg After citrate 7.5 g Na EDTA 250mg Purified water up to 250ml
But these ear drops are intended for the external auditory canal. The presence of propylene glycol will be in the middle of something.
