How to prepare Ichtyol 200 mg Prednisolone 1 mg Cameron 80 mg lidocaine 25 mg Suppo volw. 20 piece

Attempts with and without Aerosil failed, as a basis we used Novata B. Like your advice on excipients and the procedure to follow.

Answer

(in collaboration with Pharmacist A. Driessen)

First of all, a trituration 1:50 should be prepared from prednisolone because of the small not accurately weighing quantity but also to ensure the homogeneity of the Prednisolone per suppository. In addition, I would still provide additional 400 mg of milk sugar per suppository. We start by preparing the powder mixture according to the rules of the art but with extra care to rub the Cameron finely so that we obtain a smooth light brown powder mixture. In addition, the Ichtyol is weighed off in a small mortar and homogenous mixed with a third of the melted suppository mass. The powder mixture is gradually added to this mixture. A second amount of molten mass can be added so that about half of the suppository mass was used. The contents of this mortar are then mixed with the remaining molten mass in the pouring pan. After homogenous mixing, the mass can be poured into the suppository form.

Comments:

  1. I suspect that Aerosil is not absolutely necessary and is best avoided due to its large adsorption ability.
  2. theoretically there is, because of the solubility of lidocaine in fat, possibility of melting point lowering but probably that is not very large. If not, a suppository mass with elevated melting point should be used.
  3. Topical, Ichtyol Cordes is no longer available. The ammonium bithiolate, which is now delivered, corresponds to the Pharmacopoeia standards but does not have the same properties as Ichtyol Cordes. This difference causes problems in the preparation of suppositories with Ichtyol. Therefore, it is advisable to rub Ichtyol in advance with 5% (relative to the suppository weight) Arlacel C.
