Coffein solution for prematurity gives precipitation (wires)

Our Coffein solution 10mg/ml with formula: Coffein base 100mg and citric acid 100mg and aqua Purificata ad 10ml sometimes gives a thread-like precipitation. Can this be due to the shape of one of the ingredients?

Wouldn't your problem be a pH problem? And the quality of the water used?

  An alternative formula for Coffein solution contains: Caffeine anhydrous 100 mg, citric acid monohydrate 50 mg, after citrate 2 aq 83 mg and water for injections AD 10 ml. If necessary, set the pH   to 4.7  

I also found the formula: Coffein Anh 100 mg, after benzoate 100 mg and water for injections   to 10 ml.   Here I found no clear pH indication.
